Search is changing. Patients aren't typing "dentist near me" into Google like they did in 2019. They're asking ChatGPT, Perplexity, Gemini, and Copilot — and they're booking whichever practice the AI surfaces.
The dental marketing industry is, almost without exception, asleep at the wheel on this. Of 40 leading US dental marketing agencies we audited, exactly three are doing meaningful work on AEO. The rest are still selling 2018-era SEO services with a sprinkle of "AI" in the deck.
Meanwhile dentists are paying $1,500 to $10,000 per month and getting reports about impressions instead of patients, contracts that hold their domain hostage, and "managed Google Ads" with 5× markup. We talked to 100 dentists who'd been through this. We took the patterns and built their opposite.

Independent dental practices and small groups in the US — solo and 2-doctor general practices, cosmetic and orthodontic shops, oral surgeons, periodontists, and full-arch implant teams competing with ClearChoice. We don't do DSOs in the 50+ location range. We don't do offshore. We don't do mass-market commodity.
How we operate.
Thorli is in pre-launch. The team today is Karen Martin as founder and lead strategist, with a small US-based bench of contributors brought in as engagements scale, and a licensed-dentist consultant retained for any clinical content review. We are accepting a Founding Cohort of ten practices first; we will expand only when delivery quality holds. When we cap out, we cap out — and we say so.

The name.
Thorli is Marathi for "eldest sister." The eldest sister figure is the one who shows up — protective, accountable, honest. That's the agency posture we wanted to ship under. It also doesn't sound like every other dental marketing agency, which is a feature.
